Prior to attendance, and in compliance with State of Montana requirements, Carroll College requires the following documentation:
- Proof of a meningococcal (meningitis) vaccination within the last 5 years (note: not Meningitis B)
- Proof of a PPD (tuberculosis) skin test completed within one year prior to attendance (required for all nursing students or for anyone who answered “yes” to the screening questions on the immunization form)
- Proof of a completed tetanus series, including a booster within the last 10 years
- Proof of two MMR (measles, mumps, rubella) vaccinations
- Proof of completion of the polio vaccine series
Completed immunization forms are due before orientation.
