Prior to attendance, in accordance with the State of Montana requirements, Carroll College requires the following;

• Proof of a meningitis vaccination within the last 5 years (NOT meningitis B)
• Proof of a PPD (tuberculosis) skin test within one year prior to attendance
• Proof of a tetanus shot within the last 10 years and series
• Proof of 2 MMRs
• Proof of polio series

Completed immunization forms are due before orientation.

Applicants who meet certain criteria may be asked to submit a letter of recommendation as part of their application for admission. The recommendation may be a letter or this form.
