SECONDARY SCHOOL REPORT
The secondary school report must be completed by all Carroll College applicants.
RE-ACTIVATION FORM
If you applied to Carroll, but did not attend, and want to "reactivate" your application for the next semester, complete this form.
ENROLLMENT RESERVATION Return your $300 enrollment reservation deposit and the enrollment reservation form as soon as possible. Room assignments can’t take place until enrollment reservation is received. Deposits are nonrefundable after May 1.
COMMUNITY LIVING ROOMMATE INFORMATION All students living on campus are asked to return the Community Living Roommate form, which will be used when making room assignments.
HOUSING CONTRACT Students living on campus must sign and return a housing contract before moving into the residence hall.
PRIVATE HEALTH INSURANCE INFORMATION If you have health insurance through an individual, family, or state-assisted health insurance policy, you must complete and return the Personal Health Insurance Information form. Tuition and room and board billing statements are sent in June. If information about private health insurance is not received, your billing statement will include a charge for Carroll’s student health insurance.
REQUIRED STUDENT IMMUNIZATION RECORD The State of Montana requires the following immunizations: 2 MMR, DPT series and booster within last 10 years, Polio, and Tuberculosis skin test within the last year. You will not be allowed to attend classes if your completed and signed immunization records have not been returned.
RE-ADMISSION FORM If you have taken courses at Carroll previously, complete this form for re-enrollment.
NON-DEGREE STUDENT APPLICATION FORM
If you are a non-degree seeking applicant, fill out this form and return it to the Office of the Registrar.
