Parents and Athletes:
We have new forms this year. Please open the Word document and fill the form out online (except physician exam form). Once you have finished please print the completed forms, get your physical done by a physician, and send all of the forms in. If you are unable to open the Word document - open the PDF version, print the forms, complete, and send in.
Please complete all sections!
Please include a copy of the front and back of your current insurance card when you mail the information in.
