LETTER OF RECOMMENDATION FORM
Applicants to Carroll College must submit a letter of recommendation as part of their application for admission. The recommendation may be a letter or this form.
CERTIFICATE OF FINANCES
All international applicants for admission to Carroll College requiring an F-1 or J-1 Student Visa must complete this confidential statement of financial support form form before a Form I-20 or DS-2019 can be issued.
If you applied to Carroll in the last two years, but did not attend, you may "reactivate" your application for the next semester by completing this form. Admission files can only be re-activated for up to two years after the original start term, if you applied to Carroll more than two years ago, we invite you to submit a new application.
Return your $300 enrollment reservation deposit and the enrollment reservation form as soon as possible. Room assignments can’t take place until enrollment reservation is received. Deposits are nonrefundable after May 1.
COMMUNITY LIVING INFORMATION
All students living on campus are asked to return the Community Living Student Housing Agreement, which will be used when making room assignments. If you are living on campus, you will also need to select your meal plan.
PRIVATE HEALTH INSURANCE INFORMATION
If you have health insurance through an individual, family, or state-assisted health insurance policy, you must complete and return the Personal Health Insurance Information form. Tuition and room and board billing statements are sent in June. If information about private health insurance is not received, your billing statement will include a charge for Carroll’s student health insurance.
REQUIRED STUDENT IMMUNIZATION RECORD
Prior to attendance, in accordance with the State of Montana requirements, Carroll College requires proof of your MMR, DPT, and Polio vaccinations. Carroll also requires a Tetanus immunization within the past ten years, a Meningitis immunization, and a Tb skin test within the past one year. You will not be allowed to attend classes if your signed and completed immunization records have not been returned.
ADVANCED COLLEGE EXPERIENCE (ACE)
The ACE program provides the opportunity for juniors and seniors in high school to take Carroll College courses. You'll find more details on this informational PDF. Application available after May 1.
